What is a Patient Navigator?

The goal of a Patient Navigator is to improve overall disease management which results in better health outcomes for SLE and lupus nephritis (LN) patients.

About the Patient Navigator Program

The goal of this program is to improve overall disease management which results in better health outcomes for the Grady, Emory Midtown Nephritis Clinics', SLE and lupus nephritis (LN) patients, and the entire community of individuals living with lupus. 

The Navigator will be contracted by the Lupus Foundation of America, Georgia Chapter and will utilize a variety of resources to support clinic patients in the following areas:

  • Provide community support to both newly and currently diagnosed SLE/LN patients.
  • Identify, understand, and address barriers to care when possible.
  • Identify, understand, and address social determinants of health when possible.
  • Provide educational resources to help patients learn as much as possible about living with lupus.

Following up with patients based on next steps agreed upon during the prior or previous visits.  Follow up may include (but not limited to):

  • Appointment reminders
    • Adherence to treatment recommendations and testing procedures.
    • Assisting the patient with navigating insurance coverage challenges.
    • Identifying resources for patients.
    • Recording patient visit notes and recommended next steps.
  • Communication with clinic staff as needed/appropriate.
  • Tracking and recording outcomes
  • Identification of and sharing of educational resources and upcoming programs/events.
